Hi, its not that difficult but you would have to do the work yourself. Simply google for Hospitals in Chicago then check that the hospital provides Labor and Delivery/Obstetrics and Gynaecology services. Proceed to the 'Find a physician' menu on their website and call any of the physician you choose. Tell them to give you a breakdown of their charges and requirement via mail

I think i made a mistake on my ds160, i changed my surname last year, now i have a new surname, my international ID and all travel documents carry mew new name, now my question is, when filling the ds160 form, i didnt see anywhere to include my old surname, since my new documents and port isnt carrying the name. So i am curious, during istrative work after visa issuance, say they saw my old name somehow, wont i get into trouble with that? by the way I applied for F1 and i already booked my appointment for next week. In the ds 160, after the column for name, right afer it is a column for any names answered before. That's where we usually put our maiden name and wicked village names that we have dumped legally. Look at the ds 160. Its on page 1 and easy to spot.
If you are serious, you can get them to change it via the same process that you quoted.
IMO, omitting a former name is a serz ish that can be misunderstood.
Make an effort to change the form. If you are not successful, you can then tell the VO orally the moment the interview starts

Use Google or Healthgrades and get names of hospitals. Start calling and mailing them ASAP. That's your doctor and hospital's correspondence.

If u fill a new form aand email them to swap, you don't pay a new fee and retain the same date.
Aside sha: with all these mistakes why don't you take a new date and get your act together? Its better to get a later date than be refused for a silly reason
