I have not seen the rash but based on the age of your child and the culture growing Strep pyogenes, it is most likely a case of impetigo.

From the sensitivity results there, Erythromycin is the choice.

If the spots are very few or it does not look too severe you can try cleaning with topical antiseptic like Hydrogen peroxide or Povidone-iodine or applying topical antibiotic like Mupirocin. If the topical treatment fails, you can try oral Erythromycin.

The Chlorpheniramine is a sedating antihistamine and is not preferred, especially for kids. Non-sedating antihistamines like Cetirizine and Loratadine are better.
