Just get your port photo and use stepler or gum to pix it. Then photocopy it for 2 or 3 times it will look like as everyone else slip

Let me advise you those of you that your picture did not appear on your exam slip. Use computer to copy the photo on the printout slip when you ed. And paste it in the vacant space.
